1. Grade XII students from Senior High School/Vocational School/Islamic High School in the Academic Year 2022/2023
2. Minimum average grades for Mathematics and English from Grade X to Grade XI, each at least 80.
3. Photocopy of report cards from Grade X to Grade XI legalized by the school, 1 copy each;
4. 1 colored 3x4 passport photo;
5. Health Statement from Public Health Center/General Hospital;
6. Letter of Good Conduct issued by the applicant's school;
7. Photocopy of Awards or Certificates obtained from Academic and non-Academic Achievements, 1 copy each;
8. Pass all selection stages (administrative selection, academic potential test, interview, and psychological test).
